This can be caused by different factors including, physical, hormonal and muscular factors. As the uterus contacts, these contractions can cut off oxygen briefly to the muscles, causing cramping-like pain (similar to very strong period pain). The cervix (opening to the uterus) must stretch from 0 to 10 cm to let the baby pass. This stretching can cause intense pressure and pain, especially in the lower abdomen and back. The vaginal opening and perineum stretch or may tear as the baby is born. This can cause a sharp pain, especially during the pushing phase. As the baby moves down the birth canal, it puts pressure on pelvic bones, bladder and nerves and these can be painful as well.
